Please bring your insurance identification cards and any medications you are taking. If you have been under the care of another doctor, we may ask your permission to obtain a copy or transfer some of your records.

Cancellations of AppointmentsPlease call The Eye Center promptly if you are unable to attend an appointment. This time will be reallocated to another patient. If it is necessary to cancel your scheduled office appointment we require that you give at least 24 hours notice. To cancel an appointment, please call us at (803) 256-0641.
